Use this form if you want to complain about WorkSafe because:
- we have not done what we said we would
- you disagree with a decision we have made
- you are unhappy about a delay in our response
- you are unhappy with the way our staff or contractors have behaved.

Use a different form to:
- raise a health or safety concern about a business or organisation
- make a complaint about an authorisation holder to help us monitor their compliance
- make a protected disclosure about an organisation you have worked for (known as whistleblowing)
- request an internal review of a reviewable decision
